As I recall, I took the 12½% off the bill, and ten gave them about 20% in cash, and let them work it out.

Generally I tip quite heavily, on the basis that I'm a fair bit better off than the average waiter, so the total utility of them being up a fiver compared with me being down a fiver seems positive.
But I'm also pretty ruthless about not rewarding actual bad service.
